First time I see the plugin not working.

Check the image title. The 'alt' is correct, but the 'title' it's the wordpress one.
The 'Override default WordPress alt (recommended)' is checked. What about the title one?

Try disabling your other plugins and see where is the conflict.

Thanks, just did that.
Left only the Askimet plugin and didn't work either. Same thing.
Strange. Only blog that I found with that error.
