Or does the plugin alter featured images alt text and titles too, and there's something not working with my installation?
This is being called within the loop, but the image is not being displayed within the_content.
There don't seem to be any changes within the image attachments themselves (when viewed through the wordpress media admin section) although the plugin is working brilliantly when an image is inserted into the post.
I also need SEO title for featured images very urgently. I am reluctant to delve into your excellent code. Is it possible you will be adding this feature soon? Thank you.

Is there a way to do so still? You mentioned smth about premium version, could this help?

It will apply alt and text to all kind of images as long as they are part of the_content
